I would like to know, is there ever a case where a problem will come up in say, the newest release that would have otherwise not been in a previous release?

Of course. In a project of this size it is hard to test everything (even harder with so few members on the team) So there is a good chance everything wasnt tested and a silly mistake or two was never found / fixed or just blatantly ignored due to complexity and time constraints.
